We are excited to partner with you in providing white-glove services for the estates we manage. To begin working with Bravo Estates, we ask all vendors to complete our onboarding process. This ensures we have the correct information on file and that our relationship is built on mutual professionalism, trust, and clear expectations.

What You’ll Need to Complete This Form

Your business name and primary contact information
A completed W-9 (for tax purposes — SSN or EIN accepted)
A current Certificate of Insurance (COI)
Any applicable trade licenses or certifications
Your preferred invoicing details (ACH, check, etc.)
Emergency/after-hours contact information (if applicable)

📌 Important Note on File Uploads:
Our vendor form is powered by Google Forms. If you are unable to upload documents directly (because Google may require a Gmail login for uploads), you may instead email your W-9, COI, or licenses directly to inquiries@bravoestates.net. This ensures we can complete your onboarding without delays.

Upload supporting documents (or email them to inquiries@bravoestates.net if you encounter upload issues).
Check the box confirming your agreement to the Terms of Service, Privacy Policy, and Vendor Agreement.

Once complete, your information will be added to our vendor network, and you’ll be eligible for scheduling and assignments.
